What This Actually Looks Like
Here’s what usually happens:
First, an assessment is completed to determine the level of care needed.
From there, hours of care are approved, and a plan is put in place to manage those services.
Care can then be arranged at home.
Along the way, there may be calls, paperwork, and some waiting — this is where many families feel stuck.

What to Expect
- The assessment is done by the New York Independent Assessor (NYIA)
- Services are usually managed through a Managed Long-Term Care (MLTC) plan
- Once approved, care can be set up at home
